Case Manager for Adults with Intellectual Disabilities

Case Manager: Network 2 (Wilmington)

POSITION OVERVIEW

Under the supervision of the Program Coordinator, performs highly responsible duties related to the development, implementation and coordination of Individual Service Plans for those individuals served within the program.  Maintain, review and evaluate pertinent program data or information relative to each individual’s growth and development.  Maintain a leadership role in explaining and interpreting Agency policy and service principles with special emphasis given to community integration, empowerment, normalization, etc.

DUTIES AND RESPONSIBILITIES

  1. Convene and chair the Community Support Team Meetings to develop and update Individual Service Plans.
  2. Guides and leads the Community Support Team in treatment and habilitation methods.
  3. Identify and respond to gaps in service provision.
  4. Together with the Program Coordinator and Team Leaders, ensures that services are being correctly implemented and continue to meet the needs of the individuals served.
  5. Provide personal support and assistance in gaining access to vocational services, recreation and leisure activities, social experiences, necessary consultant services, etc.
  6. Advocate of the individual's rights and services.
  7. Understand and apply related regulatory standards including Illinois Department of Human Services Community Integrated Living Arrangements rules and regulations.
  8. Provide pre-service and in-service training to Independent Living Counselor’s and other staff as indicated.
  9. Emphasize such issues as choice, the attainment of daily living skills, independent functioning and economic self sufficiency in the treatment and habilitation methods and approaches being utilized.
  10. Review individual service plan on a monthly basis for each individual assigned; complete necessary forms and reports.
  11. Conduct programmatic meetings as assigned and indicated.
  12. Make suggestions for program or Agency improvements, shares ideas with Trinity colleagues.
  13. Seek opportunities for personal development; read and apply relevant clinical information.
  14. Other duties as assigned.

QUALIFICATIONS

  1. Requires a Bachelor's Degree in a Human Services field.
  2. Also, requires at least one year experience working with individuals with developmental disabilities.
  3. Requires demonstrated ability to supervise or coordinate professional, non-degreed and support staff in developing and implementing Individual Service Plans, should be able to communicate clearly, both verbally, and in writing.
  4. Service Agreements are to be completed and signed by guardians before individuals are accepted into a program. All concerns need to be addressed in the Service Agreement prior to admittance. This is not necessarily a standardized document. When areas of concern arise or when the Service Agreement needs review you should be discussing any additions or revisions with your team leader or supervisor.
  5. The physical activities for this position involve: balancing, fingering, pulling, pushing, standing, climbing, grasping, stooping, crouching, crawling, kneeling, reaching, lifting, repetitive motion, pushing wheelchairs, and lifting up to 40 pounds.
