Mission Stateme

Welcome to the Trinity Services Family Portal, a place where you can share your personal concerns or compliments regarding the care provided by our Trinity staff, as well as learn about advocacy initiatives affecting people with disabilities and mental health needs.  As a parent or guardian of a person receiving services and supports from Trinity Services, we know the well-being and comfort of your loved one is your number one priority.  The Family Portal is a secure location where you can confidently express your feelings about the care and support received by our trained personnel and receive important feedback in a timely manner.  

Concerns

As a parent or guardian, you may have specific concerns regarding the care and support of your loved one.  Trinity Services takes these concerns very seriously.  If you have a concern, simply fill out the form with your name, email address and phone number, and indicate your preference for sharing information.  Then list your concern in the space provided.  Please be as specific as possible in describing the incident or situation, including the names and location of the people involved.  Once we receive your concern, Trinity will reach out to you within 24 hours.

Compliments

Trinity staff members work hard to provide the highest quality, person-directed services and supports to people with disabilities and mental health needs so they may flourish every day.  To that end, Trinity invests thousands of hours each year educating and training our employees in the most current, most researched, and most effective practices in our field.  If you have a compliment you would like to share about an individual or group, simply fill out the form with your name, email address and phone number, and indicate your preference for sharing information.  Then list your compliment in the space provided, including the name or names of the Trinity staff who provided the care and support.  Once we receive your compliment, Trinity will share it with the appropriate personnel.

Advocacy

Here, you will find a direct link to They Deserve More, a statewide coalition of family and friends of people with intellectual and developmental disabilities, nonprofit community providers, advocacy organizations, and more, where you can take action to support important and essential legislation to protect and improve the lives of the people we support.
