Case Manager for Adults with Intellectual Disabilities (New Lenox)

Location: New Lenox, IL and surrounding areas 

Salary: $50,000/annual starting, $1,000 and $2,000 increases after training, quarterly bonuses and length of service bonuses

Schedule: Monday-Friday 8:30am-5:00pm with 2 early evening nights (11-7:30pm). 

Description:

Looking for a meaningful career supporting people with intellectual disabilities?  Check out the Case Manager position at Trinity Services. This leadership position involves supporting program participants and staff in a residential network located in the Joliet and surrounding areas. 

Responsibilities:

The Case Manager role is an exciting opportunity that will allow you to:

  • Make a difference in the lives of people with disabilities
  • Guide and lead the Community Support Team in providing individualized supports and services
  • Make suggestions and recommendations to the Community Support Team regarding any additional services and supports that may be beneficial to the person’s health and well-being.
  • Provide coaching and mentoring to the direct support staff
  • Serve as a champion in assisting people supported to progress towards and achieve their outcomes

Minimum Requirements:

  • Bachelor’s degree in sociology, special education, rehabilitation, counseling, psychology, social work, applied behavioral studies, elementary education, communicative disorders, gerontology, health education, human development
  • At least one year experience working with individuals with developmental disabilities

Physical Requirements: 

Be able to lift over 40 pounds from floor to waist unassisted, push and pull 75 pounds, complete deep knee squat, push wheelchairs up to 200 pounds, remain in a stationary position, often standing or sitting for prolonged periods, operate motor vehicles.  While performing the duties of this job, the person is regularly required to, stand, sit; talk, hear, and use hands and fingers to operate a computer and telephone keyboard reach, stoop kneel to install computer equipment.  Physical activities that could be used include: driving, balancing, pulling, pushing, standing, climbing, grasping, stooping, crouching, crawling, kneeling, reaching, lifting, and repetitive motion.
