Position: Therapist/Mental Health Professional- Community Support Team (CST)

Location: Joliet, IL

Shift: Variable weekday; 2 evening shifts per week

Salary: $40,000-$47,000

POSITION OVERVIEW:

Under the direction and immediate supervision of the Community Support Team (CST) Program Director, the Therapist/Mental Health Practitioner (MHP) provides mental health screening, intake, case management and crisis services in the Crisis Care System programs. The MHP plays a key role, providing assessment and support, promoting recovery, and working collaboratively with other staff to help people supported to develop an appropriate action plan.

CST consists of mental health rehabilitation services and supports to decrease hospitalization and crisis episodes and increase community functioning for the individual to achieve rehabilitative, resiliency and recovery goals. CST facilitates illness self-management, skill building, identification and use of adaptive and compensatory skills, identification and use of natural supports, and use of community resources. CST services will be provided in natural settings a majority of the time and service location will be based on the needs of the individuals supported.

ESSENTIAL FUNCTIONS:

  • Provides support to people experiencing mental health crisis
  • Provides supports in natural settings and location based on the needs of the individuals supported
  • Conduct required assessments when responding to crisis situations
  • Assisting individuals to complete paperwork to enroll in and maintain benefits
  • Supporting individuals during medical and psychiatric appointments
  • Assisting individuals redevelop independent living skills, such as teaching individuals how to cook healthy meals, clean, do laundry, etc.
  •  Providing crisis intervention to individuals when necessary
  • Completes Medicaid service documentation in a timely manner
  • Conducts outreach and assists in connecting to resources in the community

SKILLS AND ABILITIES:

  •  Experience either as a consumer of or facilitator of groups
  •  Strong communication skills
  • Empathic listening skills

QUALIFICATIONS:

  • Bachelor’s Degree in Psychology, Social Work, or related field
  • Experience either as a consumer of or facilitator of groups
